- Be a legal resident of 50 US or the District of Columbia
- Their age must be at least 18 years or above.
- Must not be the employees, officers and directors and their immediate family members of Pet Supplies Plus, its affiliates, subsidiaries, and associated agencies or anyone related to the promotion.
- Visit a participating Pet Supplies Plus retails stores lately during the sweepstakes period and receive a purchase receipt with a survey invitation for the online survey.

There are two ways you can take to be entered into the Pet Supplies Plus Customer Service Sweepstakes: Online Survey and Mail Request. Below are the brief descriptions of both for how you can complete them properly.
Online Entry
- Go to the survey site on your Pet Supplies Plus receipt or visit www.tellpetsuppliesplus.com to access the Neighbor Satisfaction Survey webpage.
- Enter the required pieces of information such as 4-digit Store Number, Transaction Number, Date and Time from your Pet Supplies Plus receipt. (For difficulties to locate those numbers, see the sample receipt image on right side of the page)
- Select “Yes” to ensure you are over 13 years of age (If you choose “No” you cannot proceed further to continue the survey)
- Click on the green “Start” button to begin the process.
- Answer the questions regarding your most recent experience at Pet Supplies Plus.
- Submit the required details to be eligible for the random drawing applicable to that entry period.
Mail-In Entry
If you do not wish to take the online survey or make a qualifying purchase to receive the survey invitation, you can enter Pet Supplies Plus Sweepstakes by mail. - All mail entries must be postmarked by the last day of the applicable Entry Period and received no later than the eighth day of the following month to be entered into the sweepstakes.
- Total 12 random drawings, each on or about the 10th day of the month following an entry period, will be held during the Sweepstakes.
- In each drawing one grand prize winner will be selected randomly from all valid entries to be awarded a $100 Pet Supplies Plus gift card.
